QUALIFYING REASONS FOR FMLA LEAVE - FMLA leave may be taken for the birth and care of a newborn child, placement of a child for adoption or foster care, care of a spouse, child, or parent with a serious health condition, or the employee's own serious health condition. - Military caregiver leave of up to 26 workweeks is available to an eligible employee who is the spouse, child, parent, or next of kin of a covered servicemember with a serious injury or illness. LEAVE ADMINISTRATION & EMPLOYEE OBLIGATIONS - Employees must provide 30 days' advance notice for foreseeable FMLA leave. Medical certification from a healthcare provider is required for leave based on a serious health condition. - FMLA leave may be taken as a continuous block, on an intermittent basis, or on a reduced schedule when medically necessary. BENEFITS, REINSTATEMENT & ANTI-RETALIATION - The Organization shall maintain the employee's group health insurance during FMLA leave on the same terms as during active employment. Employees are entitled to reinstatement to the same or equivalent position. - The Organization strictly prohibits retaliation, interference, or discrimination against any employee who exercises their rights under the FMLA. POLICY ADMINISTRATION & REVIEW - The HR department administers all FMLA leave requests and maintains required records. This policy shall be reviewed annually for compliance with federal and state law.
