01What is the Peer Feedback Survey?
The Peer Feedback Survey collects input from colleagues who work alongside a person day to day. Peers often see collaboration, reliability, and everyday conduct more clearly than a manager does, which makes their perspective valuable for development. The template asks raters to respond to statements about whether the colleague actively collaborates toward shared goals, is willing to help beyond their own responsibilities, follows through on commitments, and respects diverse perspectives. It also covers communication and conflict, with statements on communicating respectfully and constructively, giving useful feedback when asked, and managing disagreements professionally. Run it as part of a development review, a team health check, or a 360 process, keeping the focus on growth rather than ranking colleagues against each other.
02Why run a Peer Feedback Survey
Much of a person's real impact shows up in how they work with the people around them, and peers are best placed to describe that. Structured peer feedback replaces hallway impressions with specific, comparable observations about collaboration, dependability, and how someone handles tension. It highlights strong team players whose contributions might otherwise go unnoticed by managers, and it surfaces friction, such as missed commitments or poorly handled disagreements, early enough to coach. For the individual, seeing how colleagues experience their teamwork builds self-awareness that a manager's view alone cannot. For teams, aggregated themes point to where collaboration norms are working and where they need attention.
03What this Peer Feedback Survey covers
The template runs to roughly 20 questions grouped around collaboration, reliability, respect, and communication. On teamwork and dependability, raters respond to statements such as "This colleague actively collaborates and contributes to shared team goals," "This colleague is willing to pitch in and help others even when it falls outside their immediate responsibilities," and "This colleague follows through on commitments and delivers what they promise." Respect and inclusion appear in "This colleague respects diverse perspectives and creates space for others to contribute." The communication and conflict theme includes "This colleague communicates respectfully and constructively in all interactions," "This colleague provides useful and thoughtful feedback when asked," and "This colleague manages disagreements or tension professionally and without escalation," plus an open prompt to describe a time they went above and beyond for the team.
04How to use this free template
Copy or download the questions and load them into your survey tool, such as Google Forms, Typeform, or a review platform. For each person, invite a handful of colleagues who work with them regularly, and keep responses anonymous so feedback stays honest. No signup is required. Aggregate results before sharing so individual raters cannot be identified, and pair the summary with a development conversation. Frame the exercise as mutual growth, not a popularity contest, to keep responses fair and constructive.
